Recent CBSM updates

In September 2024, DHS made updates to the following pages in the CBSM.

Information about update

Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older

We updated the list of providers on CBSM – Resource: Customized living provider settings only allowed to serve people on the BI and CADI waivers who are age 55 and older to reflect provider enrollment status as of Sept. 1, 2024.

Lead agency employment liaison contact list

We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Anoka.
  • · Lac qui Parle.
  • · Red Lake.
  • · Renville.
  • ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    Transition services provider contact list

    We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Accord Living Services, LLC.
  • · Accord Care Services, LLC.
  • · Affinity Care Services.
  • · Allied Community Services.
  • · Elatecare, Inc.
  • · Friendship Care, LLC.
  • · Gateway Unlimited Living, LLC.
  • Assessment applicability and timelines

    We updated CBSM – Assessment applicability and timelines to make the following changes:

  • · Updated language to require an in-person assessment no later than 20 business days from the date the person requests it.
    Note: Previous policy required the assessment to take place 20 calendar days from date of request. DHS is in the process of updating the MnCHOICES application to reflect this change.
  • · Added information about starting personal care assistance (PCA) or Community First Services and Supports (CFSS) services within 60 days of the initial assessment and then starting waiver services.
  • This policy aligns with law from the 2024 Legislative Session, chapter 125, article 1, section 15.

    AC fees

    We comprehensively reviewed CBSM – Alternative Care (AC) fees and added more specific information to the InfoPac reports section.

    AC discretionary services

    We comprehensively reviewed CBSM – AC discretionary services and clarified that when completing AC Application for Discretionary Services July 1 – June 30 FY, DHS-5815 (PDF), lead agencies must describe payment rates and units of service with details of how they calculated the total estimated expenditures.

    AC

    We comprehensively reviewed CBSM – AC and made the following changes:

  • · Removed information about asset assessments for married participants from the eligibility section due to a policy update.
  • · Added a section about citizenship and immigration.
  • · Added a section about AC allocations.
  • · Added additional resources.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, go to TrainLink and search for course code “Housing_CM”.
