CLQI grant RFP
DHS seeks proposals from qualified responders for a CLQI grant to improve overall quality of life for people who receive CL services through the BI Waiver, CADI Waiver or EW.
Qualified responders will be responsible to explain how their proposal can positively affect the following domains of quality:

DHS expects to award up to $2 million in grants to successful responders from March 2025 to June 2026. One-time grants ranging from $25,000 to $250,000 will be available.

Process to submit a proposal
To submit a proposal in response to this RFP, responders must use DHS and Minnesota Board on Aging – Online grant portal.
Responders cannot submit proposals through mail, fax or email.

Deadline for submission
Responders must submit their proposal through the online application portal by 4 p.m. on Friday, Dec. 13, 2024. DHS will not accept late submissions.
