Eligibility update for HCBS

We comprehensively reviewed CBSM – Eligibility update for home and community-based services (HCBS) and made the following changes:

  • · Added information about personal care assistance (PCA) and Community First Services and Supports (CFSS).
  • · Clarified information about who completes the eligibility update.
  • · Clarified information about how to complete the eligibility update.
  • Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for Brown County.

    ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    Corporate foster care moratorium liaison contact list

    We updated information on CBSM – Corporate foster care moratorium liaison contact list for the following counties:

  • · Fillmore.
  • · Grant.
  • · Pope.
  • · Traverse.
  • Case management

    We comprehensively reviewed CBSM – Case management and made the following changes:

  • · Removed outdated COVID-19 information.
  • · Simplified the overview section.
  • · Added a link to the new page, CBSM – Care coordination.
  • · Added a link to DHS – Case manager and care coordinator toolkit.
  • Care coordination

    We posted a new page, CBSM – Care coordination, that describes:

  • · Eligibility.
  • · Responsibilities.
  • · Non-covered services.
  • · Qualifications.
  • · Billing.
  • Minnesota MA managed care programs

    We comprehensively reviewed CBSM – Minnesota Medical Assistance (MA) managed care programs and made the following changes:

  • · Added definitions for medical spenddown, institutional or long-term care spenddown and Elderly Waiver (EW) obligation.
  • · Clarified information in the programs section.
  • · Added sections about eligibility, spenddowns and disenrollment.
  • · Added and updated links to resources.
  • PolicyQuest

    We updated CBSM – PolicyQuest to remove information about searching PolicyQuest for adult protection, aging and disability-related information.

    DSD policy help

    We updated CBSM – DSD policy help to remove information about searching PolicyQuest for adult protection, aging and disability-related information.

    EAA

    We updated the service limits section of CBSM – Environmental accessibility adaptations (EAA) to reflect that the Alternative Care (AC) and EW service limit changed, effective Jan. 1, 2024.

    Transition services provider contact list

    We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Community Care Services, LLC.
  • · Faladcare, LLC.
  • · Peoples Development Services.
  • EVV Safe at Home process

    We posted a new page, CBSM – EVV Safe at Home process, that describes requirements for providers, people who receive services and caregivers.

    Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, visit TrainLink and search for course code “Housing_CM”.
