Grant opportunity for 501c3 and other non-profit social service organizations to support rural and underserved communities

DHS is accepting applications from 501c3 and other non-profit social service organizations for grants to improve access to HCBS for Medicaid-eligible people in rural and underserved racial, cultural and ethnic communities. Grantees will be responsible to:

  • · Engage with people through outreach and service development activities.
  • · Connect people to qualified HCBS providers.
  • Application details

    Use the following information to apply:

  • · Deadline: 4 p.m. on Thursday, Dec. 19, 2024.
  • · Application: To apply, refer to “DHS is seeking to connect people who are eligible for home and community culturally specific and responsive services that serve historically underserved communities throughout the state” on DHS – Open grants, RFPs and RFIs.
  • · Number of grantees: Up to 20 grants.
  • · Amount: Up to $125,000 for each grantee.
  • Eligibility

    Eligible grant applicants include 501c3 and other non-profit organizations with expertise in connecting people to HCBS services.

    DHS encourages applications from bilingual and culturally specific community-based organizations with extensive experience supporting the following people:

  • · New Americans (i.e., people born abroad and their children).
  • · LGBTQ+ populations.
  • · American Indian populations located outside of tribal nations.
  • · People whose primary language is not English.
  • · People who live outside the Twin Cities seven-county metro area (i.e., Anoka, Carver, Dakota, Hennepin, Ramsey, Scott and Washington counties).
  • · People from other historically marginalized communities.
