Forms and documents

DHS:

  • · Posted Consultation Services Provider CFSS Service Delivery Plan Cover Sheet, DHS-6893N.
  • · Updated CFSS Manual – Resource: Instructions for CFSS Assessment, DHS-6893A to clarify that when a lead agency conducts an assessment using CFSS Assessment, DHS-6893A, they must also complete CFSS Program Information and Signature Sheet, DHS-6893G (PDF).
  • For a list of all forms and documents currently available, refer to CFSS Manual – Forms and documents.

    MMIS instructions

    DHS made the following changes:

  • · Corrected an error and clarified the county/tribal nation can edit a CFSS service agreement (SA) for up to 60 days after the date in the agreement start date field on the ASA1 screen on DSD MMIS Reference Guide – Finalize a type B service agreement for PCA/CFSS.
  • · Clarified information about the PCA and CFSS assessment field definitions on DSD MMIS Reference Guide – AHC2 screen for PCA/CFSS.
  • · Clarified how to calculate total units/dollars of personal care on DSD MMIS Reference Guide – ASA3 screen overview.
  • · Added CFSS instructions on DSD MMIS Reference Guide – 45-day temporary increase of PCA/CFSS services.

    MMIS system updates

DHS corrected an error that prevented counties/tribal nations from entering an SA for a person with a level I behavior and no activities of daily living (ADL) dependencies.

DHS continues to test and fix bugs in MMIS. Counties and tribal nations can use Service Agreement and Screening Document (SASD) Support Team Portal, DHS 3754 to report issues to DHS.

    Calculator tools

DHS corrected an error in the CFSS budget model calculator that listed the wrong amount of remaining non-enhanced direct care dollars for a person with no qualifying workers.
