License transition support opportunity

DHS seeks applicants for a license transition support opportunity. DHS will distribute one-time payments to Medical Assistance (MA) disability waiver service providers who transitioned or will transition from a licensed CL setting to a licensed community residential setting under a corporate foster care moratorium exception.

For more information about license transition support, refer to Laws 2024, chapter 127, article 46, section 43 and article 53, section 2, subdivision 14.

Requirements

To be eligible to apply, providers must meet all the following conditions:

  • · Register in Minnesota’s Statewide Integrated Financial Tool System (SWIFT). For instructions, refer to MMB – SWIFT vendor resources.
  • · Select a payment method (i.e., paper or electronic) in SWIFT.
  • · Have received DHS approval for a corporate foster care moratorium exception between July 1, 2022, and Dec. 31, 2023, for each eligible setting for which they request a payment (refer to the Oct. 31, 2023, eList announcement).
  • · Either have an active community residential setting license or have a pending application to transition from a CL setting to a licensed community residential setting, under the moratorium exception.
    Note: Receiving funding does not guarantee community residential setting licensure for organizations with pending license applications.
  • · Be in good standing according to DHS – Licensing information lookup if they have an active community residential setting license.
  • · Intend to provide services at each eligible setting through Dec. 31, 2027.
  • Application deadline

    Organizations must submit their electronic application by 11:59 p.m. on March 1, 2025. DHS will not consider late or incomplete applications.

    Application instructions

    To apply, applicants must:

  • · Log in to the DSD online grant management system. For instructions to access the system for new and existing users, refer to the grant application tutorial.
  • · Select the “apply” button for the “2025 license transition support for small CL or CRS disability waiver providers’ funding opportunity.”
  • · Carefully read all information in the attestation and agreement before they submit their application.
  • · Complete a separate application for each setting that meets the eligibility criteria for the funding.
  • Applicants can access and save their applications as many times as needed until they submit them.

    After they submit, applicants will receive an email confirmation.

    Payment information

    Each eligible licensed setting can receive up to $15,000. DHS reserves the right to offer amounts that differ from the applicant’s request. Award decisions are final and not subject to appeal.

    DHS will:

  • · Ask organizations for clarifications, if necessary, before we distribute payments. We encourage organizations to promptly respond to clarification requests.
  • · Begin payment distribution on or after April 1, 2025, through SWIFT using the provider’s SWIFT supplier ID number.
  • · Email applicants to inform them of payment.
  • Organizations may only use the payment for expenses that:

  • · They incur after the date DHS approved the moratorium exception.
  • · Support their transition to provide services in a licensed community residential setting.
  • · Cover technical assistance to adapt business models and meet policy and regulatory guidance to transition to provide services in a licensed community residential setting.
