In February 2025, DHS made updates to the following pages in the CBSM.

Lead agency employment liaison contact list

We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Aitkin.
  • · Cottonwood/DVHHS.
  • · Jackson/DVHHS.
  • · Meeker.
  • · Sibley.
  • AC

    We updated CBSM – Alternative Care (AC) to make the following changes:

  • · Updated the legal authority.
  • · Clarified information in the covered services section.
  • · Added transitional services to the covered services section and removed it from the non-covered services section.
  • · Moved estate recovery to its own section.
  • Transitional services – AC and EW

    We updated CBSM – Transitional services AC and Elderly Waiver (EW) to make the following changes:

  • · Updated the page title and legal authority and eligibility sections to include AC.
  • · Added information to clarify that the managed care organization – unforeseen circumstances section applies to EW only.
  • Rate methodologies for AC, ECS and EW service authorization

    We updated CBSM – Rate methodologies for AC, Essential Community Supports (ECS) and EW service authorization to include AC transitional services in the market rates section.

    Customized living

    We updated the service plan section of CBSM – Customized living (including 24-hour customized living) to add information about provider participation in the assessment (EW only).

    CSG transition to CFSS

    We updated the eligibility section of CBSM – Consumer Support Grant (CSG) transition to Community First Services and Supports (CFSS) to explain that a person must not participate in the Family Support Grant (FSG) to be eligible for CSG.

    ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    IHS

    We comprehensively reviewed CBSM – Individualized home supports (IHS) and made the following changes:

  • · Added a definition of direct supervision.
  • · Created a new section about service options to clarify when to authorize each option.
  • · Created a new section about location of services.
  • · Clarified information in the non-covered services and service amount sections.
  • These updates did not change policy.

    Positive support services staff levels

    We updated CBSM – Positive support services staff levels to make the following changes:

  • · Updated the waiver positive support analyst section to reduce the years of supervised experience from four to two and to expand the degree qualifications to include nursing.
  • · Updated the waiver positive support specialist section to expand the degree qualifications to include nursing.
  • These changes are part of the recently approved disability waiver amendments and reflect recent changes to Minn. Stat. §245D.091.

    Waiver/AC reimbursement for unforeseen circumstances

    We comprehensively reviewed CBSM – Waiver/AC reimbursement for unforeseen circumstances and made the following changes:

  • · Updated the legal authority.
  • · Added clarifying information in the eligibility section.
  • · Clarified information throughout the page.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS).
