Lead agency instructions to avoid gaps in services

DHS created instructions that lead agencies must follow to avoid gaps in services for people who need additional time to complete the transition from PCA/CSG to CFSS.

Applicability

Lead agencies can enter six additional months of PCA/CSG services for a person who already has a six-month PCA/CSG service agreement for an assessment conducted on or after Oct. 1, 2024, if the person meets at least one of the following criteria:

  • · The person’s service agreement will end in two months or less.
  • · The lead agency determines the person is likely to require more time to complete the transition from PCA/CSG to CFSS due to unique circumstances (e.g., the person also uses home care nursing [HCN], the person has a language barrier).
  • Lead agency instructions

    For detailed instructions, refer to:

  • · Lead agency instructions for people who need additional time to complete the transition from PCA to CFSS section of CFSS Policy Manual Assessment for PCA/CFSS services.
  • · County/tribal nation instructions for people who need additional time to complete the transition from CSG to CFSS section of CBSM CSG transition to CFSS.
  • Training

DHS updated CFSS Transition Timelines with an example of this change.

    More information

For questions about potential gaps in services, people who receive services and providers can contact their lead agency if the person meets the criteria in the applicability section above.

For more information about the transition from PCA/CSG to CFSS, refer to CFSS Policy Manual Transition from PCA and CSG to CFSS.
