Additional guidance on 2025 rate adjustments for transitional services in MMIS

DHS is providing additional guidance and clarification about the purchasing limits and service agreement process in MMIS for 2025 rate adjustments for transitional services for disability waivers.

As of May 7, 2025:

  • · DHS fixed an MMIS programming issue for exception code 635 (i.e., transitional service limit exceeded) related to transitional service line items (i.e., T2038, T2038 U1 and T2038 U2) for disability waivers as announced in the May 6, 2025, eList announcement.
  • · The Jan. 1, 2025, rate increase for transitional services applies to all transitional service lines effective on or after Jan. 1, 2025, as described in the Nov. 12, 2024, eList announcement.
  • MMIS users can adjust the rates to the limits described below:

  • · Line items with start dates before Jan. 1, 2025: Transitional services cannot exceed a total of $3,000, including the following limits for each service:
    Moving expenses (T2038): Maximum total of $3,000.
    Essential furniture (T2038 U1): Maximum total of $1,000.
    Household supplies (T2038 U2): Maximum total of $300.
  • · Line items with start dates on or after Jan. 1, 2025: Transitional services cannot exceed a total of $5,000, including the following limits for each service:
    Moving expenses (T2038): Maximum total of $5,000.
    Essential furniture (T2038 U1): Maximum total of $1,500.
    Household supplies (T2038 U2): Maximum total of $500.
  • There are no exceptions to these limits.

    Examples

    If a person had transitional services (T2038) lines open in 2024 and the provider billed, the person is allowed to purchase up to $3,000 for that waiver span.

    If a person had transitional services (T2038) lines open in 2024 and they did not move and did not make purchases for a move in 2024, MMIS users should deny the 2024 lines and add new lines in 2025.

    Note: If the provider already billed on a line item for transitional services (T2038) lines effective on or after Jan. 1, 2025, MMIS may block the MMIS user from changing the entry in the requested total amount (REQ TOT AMT) field. If this occurs, the MMIS user should use Service Agreement and Screening Document (SASD) Support Team Portal, DHS-3754 to ask DHS to make the change.
