Information about update

Transition services provider contact list

We updated information on CBSM – Transition services provider contact list for the following providers:

  • · Alpha Home Health Inc.
  • · Community Outreach Services LLC.
  • · Love Investments.
  • · Open Arms Community Outreach.
  • Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for Carlton County.

    Corporate foster care moratorium liaison contact list

    We updated information on CBSM – Corporate foster care moratorium liaison contact list for the following counties:

  • · Mille Lacs County.
  • · Stevens County.
  • ICS settings

    We updated the list of settings on CBSM – Resource: Integrated community supports (ICS) settings.

    Transitional services – BI, CAC, CADI and DD

    We updated the service amount section of CBSM – Transitional services – Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) and Developmental Disabilities (DD).

    AC conversion case management

    We comprehensively reviewed CBSM – Alternative Care (AC) conversion case management and updated the organization of the content to align with other CBSM pages.

    Disability Hub MN

    We comprehensively reviewed CBSM – Disability Hub MN and clarified information throughout the page to better explain Disability Hub MN’s role.

    AC

    We updated CBSM – AC to add personal emergency response systems (PERS) and goods available through Community First Services and Supports (CFSS) to the non-covered services section.

    RSSC

    We posted a new page, CBSM – Residential support service criteria (RSSC), to describe the new requirements that are effective July 1, 2025.

    Customized living (including 24-hour customized living

    We updated CBSM – Customized living (including 24-hour customized living) to add an eligibility requirement (BI and CADI only) for RSSC effective July 1, 2025.

    Community residential services

    We updated CBSM – Community residential services to add an eligibility requirement for RSSC effective July 1, 2025.

    IAR

    We posted a new page, CBSM – Initial assessment review (IAR) for home and community-based services (HCBS), to describe the new activity type.

    Assessment applicability and timelines

    We updated CBSM – Assessment applicability and timelines to add information about initial assessment validity extension and IAR.

    Eligibility update

    We updated CBSM – Eligibility update for HCBS to add a new section that explains the transition to the IAR. Lead agencies will no longer be able to use eligibility updates for assessments completed on or after July 1, 2025.

    MnCHOICES

    We updated CBSM – MnCHOICES to add a reference to the IAR.

    EAA

    We updated CBSM – Environmental accessibility adaptations (EAA) and made the following changes:

  • · Clarified information in the service limits section.
  • · Updated the home modifications – non-covered services section to add that AC and EW do not cover EAA provided in settings owned or leased by waiver service providers.
  • · Removed “foster care home” as an example of a primary home for AC and EW.
  • · Removed “residential setting’s vehicle” as an example of a primary vehicle for AC and EW.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, go to TrainLink and search for course code “Housing_CM”.
