Disability Hub MN grant opportunity

DHS is accepting applications from interested parties for a grant opportunity to run and staff the Disability Hub MN contact center with support from DHS. The selected responder will:

  • · Be committed to equity, inclusion and person-centered, unbiased support for people with disabilities in Minnesota.
  • · Help people access accurate information, useful resources and support that reflects their needs and choices.
  • · Deliver information, resources and support without bias or conflicts of interest.

    • Application details

    Use the following information to apply:

  • · Deadline: 4 p.m. on Aug. 1, 2025.
  • · Application: To apply, refer to DHS – Open grants, RFPs and RFIs – DHS is seeking for grantee to host the Disability Hub.
  • · Number of grantees: One.
  • · Amount: Up to $550,000 for transition planning and hiring during year one (Dec. 1, 2025, to June 30, 2026) and up to $3,135,633 for full implementation during year two (July 1, 2026, to June 30, 2027).
    Note: DHS may extend the contract up to a total of five (5) years. If DHS extends the contract, we plan to award up to $3,135,633 per year to the selected responder.

    • Eligibility

    Eligible responders must agree to:

  • · Have a physical presence in Minnesota.
  • · Understand the needs of the local community.
  • · Follow the Disability Hub MN standards and assurances, which define the essential components, values and quality expectations for service delivery. These standards guide Disability Hub MN’s operation to ensure it remains a trusted, person-centered and unbiased resource for people with disabilities across Minnesota.
