Purpose: To announce a request for proposal (RFP) for interested businesses to apply to become CFSS consultation services providers

Opportunity to become a consultation services provider

DHS invites applications from businesses interested in becoming a CFSS consultation services provider.

Application details

Use the following information to apply to become a CFSS consultation services provider:

  • · Deadline: The deadline for the current RFP is 4 p.m. on Aug. 29, 2025.
    Note: DHS will continue to accept proposals until June 30, 2026, but we will not review proposals submitted after the current RFP deadline (i.e., 4 p.m. on Aug. 29, 2025) until after June 30, 2026.
  • · Online application: To submit a proposal in response to this RFP, responders must use the DHS – DSD Online Portal. New users must select “Create New Account” to complete the registration process. Existing users must enter their email address and password to log in. If you need to reset your password, select “Forgot your password?” and follow the instructions. For help with the online portal, refer to Foundant Technologies – Grant Lifecycle Manager (GLM) Applicant Tutorial.
    Note: Responders cannot submit proposals through mail, fax or email.
  • · Timeline: Consultation services providers may start to provide services after:
    1. DHS reviews and approves their application.
    2. The provider completes required DHS training.
    3. The provider enrolls with DHS as a Minnesota Health Care Programs (MHCP) provider.

    CFSS consultation services providers:

  • · Support people who receive CFSS services.
  • · Educate them about CFSS and their options.
  • · Help them with their CFSS service delivery plan, if desired.
  • · Review their CFSS service delivery plan and help make needed edits.
  • · Submit the plan to the person’s lead agency for approval.
