Purpose: To announce the upcoming implementation of the Notice of Action: Explanation of Changes to Your Support Plan in the MnCHOICES application

MnCHOICES Notice of Action upcoming implementation

DHS is working to:

· Add the Notice of Action: Explanation of Changes to Your Support Plan to the MnCHOICES application.

· Update CBSM – Notice of Action.

· Add resources on how to complete the Notice of Action: Explanation of Changes to Your Support Plan to the MnCHOICES help center.

We expect to add the Notice of Action: Explanation of Changes to Your Support Plan to the MnCHOICES application in August 2025. We will send another eList announcement to notify lead agencies when it is available.

Lead agency actions

When the Notice of Action: Explanation of Changes to Your Support Plan is in the MnCHOICES application, counties and tribal nations must:

· Use the Notice of Action: Explanation of Changes to Your Support Plan in the MnCHOICES application to provide notice to a person and their legal representative (as applicable).

· Send the person and their representative the language block found in the MnCHOICES help center with the Notice of Action: Explanation of Changes to Your Support Plan.

Note: Managed care organizations (MCOs) will continue to use their own form to provide notice to a person.
