Purpose: To announce the implementation of the Notice of Action: Explanation of Changes to Your Support Plan into the MnCHOICES application

MnCHOICES Notice of Action implementation

As of Aug. 21, 2025, DHS:

· Added the Notice of Action: Explanation of Changes to Your Support Plan into the MnCHOICES application.

· Added the MnCHOICES Notice of Action practice guide to the MnCHOICES help center.

DHS is working to update CBSM – Notice of action.

We announced the MnCHOICES Notice of Action implementation in the Aug. 5, 2025, eList announcement.

Lead agency actions

Counties and tribal nations must:

· Use the Notice of Action: Explanation of Changes to Your Support Plan in the MnCHOICES application to provide notice to a person and their legal representative (as applicable).

· Send the person and their representative the language block found in the MnCHOICES help center with the Notice of Action: Explanation of Changes to Your Support Plan.

Note: Managed care organizations (MCOs) must continue to use their own form to provide notice to a person.
