RFP for grantees to support people with disabilities moving out of congregate settings to their own homes

DHS is accepting proposals from qualified applicants for a grant opportunity to support people with disabilities who want to move out of congregate settings into their own homes.

Application details

Use the following information to apply:

· Deadline: 4 p.m. on Wednesday, Oct. 15, 2025

· Application: To apply, refer to DHS – Open grants, RFPs and RFIs.

· Timeline: This is one-time funding that expires on June 30, 2027.

· Number of grant contracts: DHS expects to award 5 to 10 grant contracts.

· Amount: DHS expects the funding to range from $100,000 to $300,000 per contract.

Note: The number of grants and amount of funding per grant depends on the number and quality of applications we receive.
