CFSS implementation updates

DHS began the transition from PCA and CSG to CFSS on Oct. 1, 2024. We will provide regular updates throughout the transition.

Policy manual

DHS updated CFSS Manual – CFSS service delivery plan development and approval process with instructions to temporarily authorize services for a person already receiving CFSS services at the time of their reassessment to avoid a gap in services.

Forms and documents

DHS updated CFSS Program Information and Signature Sheet, DHS-6893G (PDF) to align with MnCHOICES support planning requirements.

DHS posted Temporary CFSS Individual Service Delivery Plan Approval, DHS-6893L (PDF), a new form lead agencies must use to temporarily authorize previously approved services for a person already receiving CFSS services at the time of their reassessment to avoid a gap in services.

MMIS instructions

DHS updated:

  • · DSD MMIS Reference Guide – Finalize a type B service agreement (SA) for PCA/CFSS to clarify counties and tribal nations must enter a consultation services (T1023) line on a PCA/CFSS SA before approving it to allow them to edit it within 183 days of the approval date.
  • · DSD MMIS Reference Guide – MMIS transition from PCA and CSG to CFSS to update the phase 3 instructions for counties/tribal nations.

    MMIS system updates

    DHS continues to test and fix bugs in MMIS. Counties/tribal nations can use Service Agreement and Screening Document (SASD) Support Team Portal, DHS 3754 to report issues to DHS.
