Change to MnCHOICES remote reassessment policy for disability waivers

Effective Jan. 1, 2026, DHS will implement a change to the MnCHOICES remote reassessment policy. The new policy applies to:

  • · Brain Injury (BI) Waiver.
  • · Community Access for Disability Inclusion (CADI) Waiver.
  • · Community Alternative Care (CAC) Waiver.
  • · Developmental Disabilities (DD) Waiver.
  • New remote reassessment policy

    For annual reassessments occurring on or after Jan. 1, 2026, for people on BI, CAC, CADI and DD waivers, remote reassessments conducted by interactive video or telephone may substitute for in-person reassessments for four consecutive reassessments (including the 65th birthday reassessment), followed by an in-person reassessment in the fifth year.

    Lead agency actions

    After Jan. 1, 2026, lead agencies must review past assessments to determine if a person meets the requirements to complete their next reassessment remotely.

    Applicability

    The new remote reassessment policy:

  • · Only applies to disability waivers (i.e., BI, CAC, CADI and DD waivers).
  • · Will replace the current disability waiver policy that allows remote reassessments to substitute for two consecutive reassessments (including the 65th birthday reassessment), followed by an in-person reassessment in the third year.
  • · Does not apply to Alternative Care (AC), Essential Community Supports (ECS), Elderly Waiver (EW) or Community First Services and Supports (CFSS).
    Note: The remote reassessment policies for AC, EW and ECS (i.e., remote reassessments may substitute for one annual reassessment, followed by an in-person reassessment in the second year) and CFSS (i.e., remote reassessments may substitute for two consecutive reassessments, followed by an in-person reassessment in the third year) will remain the same.
  • Background

    DHS made this change because the Minnesota Legislature amended Minn. Stat. §256B.0911, subd. 24 during the 2025 legislative session.
