Purpose: To announce changes to MnCHOICES Assessment Summary Worksheet, DHS-6791A

MnCHOICES Assessment Summary Worksheet, DHS-6791A upcoming changes

DHS made the following changes related to MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF):

1. DHS updated MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF) to:

  • · Explain the worksheet is for initial assessments.
  • · Remove Housing Stabilization Services because the program ended Oct. 31, 2025.
  • · Simplify the information to increase efficiency for counties/tribal nations.
  • 2. Starting Nov. 17, 2025, DHS only will require counties/tribal nations to give the person a hard copy of MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF) for initial MnCHOICES assessments. Certified assessors may still provide the form to people receiving reassessments, if desired. We are making this change to help increase efficiency for counties/tribal nations. For more information, refer to the county/tribal nation actions section below.

    3. DHS will not print more carbon copies of MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF) when our supply runs out. Counties/tribal nations will have to print their own copies of the form.

    Applicability

    The changes to MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF) only apply to counties/tribal nations. Managed care organizations (MCOs) do not use this form.

    County/tribal nation actions

    Counties/tribal nations must:

  • · Continue using their current supply of carbon-copy versions of MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF).
  • · Order additional carbon copies from DHS while they are still available. To order carbon copies, refer to DHS bulk order site – DHS and DCYF forms.
  • · Print the updated MnCHOICES Assessment Summary Worksheet, DHS-6791A (PDF) when DHS’ supply of carbon copies is gone.
  • · Give the person a print copy of the form during their initial assessment.
    Note: Effective Nov. 17, 2025, counties/tribal nations no longer need to give the person a copy of the form during reassessments.
