Purpose: To alert users that some online forms will be unavailable during a software upgrade
Effective: 6 to 8 p.m. on Wednesday, Dec. 17, 2025

Some online forms will be offline during software upgrade Dec. 17, 2025

The following forms will be offline from 6 to 8 p.m. on Wednesday, Dec. 17, 2025:

· Consumer Directed Community Supports (CDCS) Budget Exception Request Application, DHS-6633.

· Extension Request for Out-of-Home Waiver Crisis Respite in MDH-Licensed Hotel, DHS-8633.

· Early Intensive Developmental and Behavioral Intervention (EIDBI) Referral Form, DHS-6751S.

· Remote Support Exception Form, DHS-7759A.

· Monitoring Technology Approval Request, DHS-6789A.

· OBRA Level II Evaluative Report for People with Developmental Disabilities or Related Conditions, DHS-4248.

· Reopening Someone Who is 65 Years or Older to a Disability Waiver, DHS-7759D.

· Subminimum Wage Reporting Tool.

User actions

To avoid losing the information in an in-progress form, save or close the form before 6 p.m. on Dec. 17, 2025.

We expect the upgrade to be complete by 8 p.m. on Dec. 17, 2025. Once the upgrade is complete, users will be able to continue form use and submission. We will send another eList announcement if that is not the case.
