To: People who receive personal care assistance (PCA), Consumer Support Grant (CSG) or Community First Services and Supports (CFSS), their families and friends and other interested parties

Purpose: To announce two upcoming in-person CFSS information sessions in Crow Wing County

CFSS information sessions

DHS and Crow Wing County will host two in-person CFSS information sessions for people who receive services. Each session will include:

· Basic information about CFSS (e.g., how to choose a CFSS service model, how to develop a service delivery plan).

· Time for people to ask DHS personnel questions.

· Opportunities for people to connect directly with consultation service providers.

Session details

Use the following information to attend either of the information sessions:

· Date: Thursday, Jan. 29, 2026

· Times: Two sessions from 11 a.m. to 12 p.m. and 4 to 5 p.m.

· Location: Upstairs at the Lakes Area Habitat for Humanity Restore, 415 Eighth Ave. NE, Brainerd, MN 56401.

· Registration: To register, refer to Microsoft Office form – Crow Wing County CFSS info session registration.

Note: The information sessions are the same, so people only need to attend one session.

Target audience

DHS developed this information session for people who receive services and want to learn more about CFSS, including:

· People who receive PCA or CSG services and will transition to CFSS.

· People newly assessed as eligible for CFSS.

· People who might receive CFSS services in the future.

Note: Lead agencies and providers are not the target audience for this information session. For more information and CFSS resources, lead agencies and providers should refer to CFSS Policy Manual and CFSS Policy Manual – Transition from PCA and CSG to CFSS.
