DSD updated RMS Worksheet – Day and Employment Services, DHS-6790M to include fields for the following additional information:
Provider actions
As of Jan. 1, 2026, providers must use the updated version of RMS Worksheet – Day and Employment Services, DHS-6790M for renewals and service changes. Providers must fill out all applicable sections.
Case manager actions
As of Jan. 1, 2026, case managers must verify all sections of RMS Worksheet – Day and Employment Services, DHS-6790M are complete before they authorize services.
DHS actions
DHS will use RMS Worksheet – Day and Employment Services, DHS-6790M to begin gathering data about wages and hours worked under prevocational services. We are still determining how we will track wages and hours worked under prevocational services. We will send an eList announcement to share more information when we finalize the process.
