Purpose: To announce updates DHS made to the Community-Based Services Manual (CBSM) in January 2026
Contact: For Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) and Developmental Disabilities (DD) waivers, use the DSD Contact Form. For Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW), email DHS.AASD.HCBS@state.mn.us. For Medicaid Management Information System (MMIS) service agreements and screening documents, refer to Service Agreement and Screening Document (SASD) Support Team.

We updated information on CBSM – Transition services provider contact list for Housing Clinic Services, LLC.

Assisted living facilities licensed by MDH and enrolled to deliver customized living by DHS

We updated CBSM – Assisted living facilities licensed by MDH and enrolled to deliver customized living by DHS to clarify how to use the provider list.

AASD lead agency resources

We comprehensively reviewed CBSM – AASD lead agency resources and made the following changes:

  • · Removed and replaced outdated links.
  • · Added contact information for the Minnesota Adult Abuse Reporting Center.
  • · Clarified policy resources.
  • · Added links to DHS – Case manager and care coordinator toolkit, Minnesota Assisted Living Report Card, Minnesota Nursing Home Report Card, Minnesota Department of Health Assisted Living Licensure and DHS MnCHOICES.
  • Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for the following counties:

  • · Wadena.
  • · Winona.
  • SNV

    We comprehensively reviewed CBSM – Skilled nursing visit (SNV) and made the following changes:

  • · Clarified that the medical providers who can order SNVs are physicians, physician assistants (PAs) or advanced practice registered nurses (APRNs).
  • · Clarified information about assessments and authorization.
  • · Clarified that authorizations must list the number of registered nurse (RN) visits and licensed practice nurse (LPN) visits.
  • Remote support

    We made the following updates to CBSM – Remote support:

  • · Removed outdated information.
  • · Updated AC, EW and ECS applicability.
  • · Added service-specific requirements for adult day services.
  • · Updated the services that require separate MMIS service agreement line item authorizations for in-person and remote support under AC, ECS and EW.
  • EIDBI benefit

    We comprehensively reviewed CBSM – Early Intensive Developmental and Behavioral Intervention (EIDBI) benefit and made the following updates:

  • · Moved the EIDBI benefit definition into a definitions section.
  • · Updated and clarified information in the overview section.
  • · Added clinical supervision and service delivery requirements and licensure information to the provider standards and qualifications section.
  • · Clarified information throughout the page.
  • ECS

    We comprehensively reviewed CBSM – ECS and made the following changes:

  • · Updated the ECS definition.
  • · Added the purpose section.
  • · Clarified information in the eligibility section.
  • · Removed the assessment section.
  • · Updated the authorization section to focus on managed care organizations.
  • · Updated information in the tools section.
  • · Clarified information throughout the page.
  • Resource: Customized living component service definitions and guide for computing time for rate-setting tools

    We updated information on CBSM – Resource: Customized living component service definitions and guide for computing time for rate-setting tools about:

  • · Covered component services for congregate meal breakfast prep – also for lunch, supper and snack prep.
  • · Non-covered component services and additional clarification for continence care.
  • Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. This course also discusses different moving supports, services and tools to help people plan for successful moves, including information about Housing Stabilization Services (HSS). To find the course, go to TrainLink and search for course code “Housing_CM”.
