Purpose: To announce a request for proposal (RFP) for the Customized Living Quality Improvement (CLQI) grant program
Submission deadline: 4 p.m. on April 17, 2026

CLQI grant RFP

DHS is accepting proposals from qualified responders for a CLQI grant to improve overall quality of life for people who receive CL services through the BI Waiver, CADI Waiver or EW.

Qualified responders must explain how their proposal can positively affect the following domains of quality:

  • · Person’s quality of life.
  • · Person and their family’s satisfaction.
  • · Safety.
  • · Person’s health outcomes.
  • · Staff.
  • · Physical and social environment.
  • · Service availability.
  • · Core values and philosophy.
  • · Care services and integration.

    RFP details

    Use the following information to apply:

  • · RFP details: To review the RFP, refer to DHS – Open grants, RFPs and RFIs.
  • · Grant amount: One-time grants ranging from $25,000 to $250,000 will be available. DHS expects to award up to $1 million in grants to successful responders.
  • · Timeline: July 2026 to June 2028
  • · How to apply: To submit a proposal in response to this RFP, responders must use DHS and Minnesota Board on Aging Online grant portal. Responders cannot submit proposals by mail, fax or email. For more information to help navigate the portal, email dhs.aasd.hcbs@state.mn.us.
  • · Submission deadline: 4 p.m. CT on Friday, April 17, 2026. DHS will not accept late submissions.
