MnCHOICES help desk contact form updates

Effective March 18, 2026, lead agency MnCHOICES mentors can use the updated MnCHOICES Help Desk Contact Form, DHS-6979. DHS made the following improvements to the form to increase efficiency based on feedback from lead agencies.

Issue description field

We added the issue description field so lead agencies can use the form to report critical issues (e.g., work stoppage, issues impacting many users). This will help DHS identify timely, critical issues and escalate issues to the vendor when necessary.

Person ID field

We added the person ID field to proactively collect this information for all question types. This field is required. DHS uses this information to locate the form ID in MnCHOICES.

Form ID field

We updated the doc ID field to call it the form ID field to match MnCHOICES.

Question type – technical

We removed version, operating system and device selections.

Question type – policy or practice

We removed the long-term services and supports (LTSS) improvement tool and other: housing stabilization services selections because these options no longer apply.
