Grants available to HCBS providers that support rural and underserved communities

DHS is accepting proposals for provider capacity-building grants to support HCBS service provider organizations. These grants will expand their ability to serve older adults and people with disabilities in rural and underserved communities.

Use the following information to apply:

  • · Deadline: 4 p.m., Friday, May 22, 2026
  • · Application: To apply, refer to DHS – Open grants, RFPs and RFIs – Provider capacity grants for rural and underserved communities: round 4.
  • · Amount: Up to $100,000
  • Eligibility

    Eligible grant applicants include current HCBS provider organizations that want to expand their existing services or increase service capacity.

    The state is particularly interested in organizations that:

  • · Serve the LGBTQ+ population.
  • · Serve American Indian populations located outside of Tribal Nations.
  • · Provide language supports and culturally specific services.
  • · Implement positive supports.
  • · Support people with complex needs.
  • Background information

    DHS wants to increase the total number of HCBS providers in Minnesota and the number of HCBS providers that offer culturally specific services.
