To: Lead agency staff who work with people who receive consumer directed community supports (CDCS)

Purpose: To announce updates to the reassessment process for people receiving CDCS on disability waivers
Effective: Immediately

Reassessment process updates for people receiving CDCS on disability waivers

DHS updated the reassessment instructions for people receiving CDCS on the following waivers:

  • · Brain Injury (BI).
  • · Community Alternative Care (CAC).
  • · Community Access for Disability Inclusion (CADI).
  • · Developmental Disabilities (DD).
  • When the county/Tribal Nation completes the person’s reassessment, they must provide the person with their most recent available budget in the Waiver Management System (WMS). This allows the person to:

  • · Work on a CDCS community support plan while they wait for their updated annual budget.
  • · Avoid gaps in service.

    No changes to reassessments for people receiving CDCS on AC or EW

    For people receiving CDCS on AC or EW, lead agencies must continue to use the current instructions in the process for people who receive a reassessment – AC and EW section of CDCS Manual – CDCS process and procedure.

    The person’s budget is based on their case mix. This information is readily available to avoid gaps in services.
