Lead agency continuity of care meetings

DHS invites lead agency staff and leadership to large-group discussions on Aug. 14 and 28 that will focus on strengthening our partnership efforts to ensure all Minnesotans have their needs met. We will discuss continuity of care for people served by disenrolled providers, as well as ongoing engagement with providers to help them revalidate.

Note that DHS is shifting these meetings from weekly to twice per month in August. They are an opportunity to:

· Share updates on current processes and challenges.

· Discuss strategies to ensure uninterrupted care for affected people.

· Collaborate on solutions and align next steps across teams.
