Purpose: To announce updates DHS made to the Community-Based Services Manual (CBSM) in August 2026
Contact: For Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) and Developmental Disabilities (DD) waivers, use the DSD Contact Form. For Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW), email DHS.AASD.HCBS@state.mn.us. For Medicaid Management Information System (MMIS) service agreements and screening documents, refer to Service Agreement and Screening Document (SASD) Support Team.

In August 2026, DHS made updates to the following pages in the CBSM.

Information about update

HHA services

We comprehensively reviewed CBSM – Home health aide (HHA) services and made the following changes:

  • · Added a definition of medical review agent.
  • · Updated the covered services section to explain that HHA services must be ordered by a physician, physician assistant or advanced practice registered nurse every 60 days.
  • · Added a section about covered and non-covered locations.
  • · Updated the provider standards and qualifications section to explain the home health agency determines if an employee is qualified to provide HHA services.
  • · Clarified information throughout the page.

    • Institutional LOC

    We comprehensively reviewed CBSM – Institutional level of care (LOC) and made the following changes:

  • · Added “Institutional” to the page title.
  • · Clarified information about the page’s applicability.

    • Lead agency employment liaison contact list

    We updated information on CBSM – Lead agency employment liaison contact list for Clearwater County.

    Assisted living facilities licensed by MDH and enrolled to deliver customized living by DHS

    We updated the list of providers on CBSM – Assisted living facilities licensed by MDH and enrolled to deliver customized living by DHS.

    Note: This page applies to customized living through BI and CADI waivers only.

    CL (including 24-hour CL)

    We updated CBSM – Customized living (CL) (including 24-hour CL) and made the following changes:

  • · Added “CL services provided by relatives” to the non-covered services section.
  • · Added information about personal emergency response systems (PERS) and monitoring technology in CL settings for people on BI and CADI waivers.

    • 24-hour emergency assistance

    We updated CBSM – 24-hour emergency assistance to add information about PERS and monitoring technology in CL settings for people on BI and CADI waivers.

    Family residential services

    We updated the authorization, rates and billing section of CBSM – Family residential services to add a link to RMS User Manual – Family residential services: DWRS tiered-rate services.

    OBRA Level II evaluative report for people with developmental disabilities

    We comprehensively reviewed CBSM – OBRA Level II evaluative report for people with developmental disabilities and made the following changes:

  • · Simplified the organization of the page to make the process easier to follow.
  • · Clarified information about the annual requirement.

    • Transition services provider contact list

    We updated CBSM – Transition services provider contact list and made the following changes:

  • · Updated information for Accord Living Services, Ace Community Connect and Love Investments.
  • · Added Superior Health and Human Services.
  • · Removed Global Health Link, Inc.

    • Corporate foster care moratorium liaison contact list

    We updated information on CBSM – Corporate foster care moratorium liaison contact list for Chisago County.

    Training

    AASD

    AASD and the Minnesota Board on Aging offer online courses and training materials for staff from counties, tribal nations, managed care organizations, providers and other interested parties about how to better serve and support older adults.

    All courses are available in the Older Adult Programs Learning Center in TrainLink.

    DSD

    Lead agency staff can take the On-Demand HCBS Disability Waiver Foundations course to learn the basics about the four disability waivers, including eligibility and covered services.

    The Supporting A Person’s Move: A Case Manager’s Role course guides case managers through their responsibilities in supporting a person to find a new home. To find the course, go to TrainLink and search for course code “Housing_CM”.
