Use the following information to complete the survey:

· Survey opens: Oct. 28, 2026

· Survey closes: Dec. 18, 2026

· How to participate: To complete the online survey and review instructions and FAQs, refer to Guidehouse – Minnesota DHS rate study.

· Survey partner: Guidehouse, Inc. is DHS’ contractor for this project.

Instructions for providers who completed a survey in 2025

Providers may have completed an earlier version of this survey in early 2025. These organizations can either:

· Email Guidehouse at MNDHSRateStudy@Guidehouse.com to request a copy of their previously submitted survey and update it.

· Complete a new survey at Guidehouse – Minnesota DHS rate study.

Survey purpose

This survey allows providers to help inform the components and factors of the rate methodology for critical services.

Next steps

Guidehouse will email providers the survey, project website and training and technical assistance information.
