CFSS implementation updates

DHS began the transition from PCA and CSG to CFSS on Oct. 1, 2024. We will provide regular updates throughout the transition.

Policy manual

DHS posted a new page, CFSS Manual – Temporary CFSS service delivery plan approval, and made the following changes:

  • · Moved information about temporary approval of CFSS service delivery plans from CFSS Manual – CFSS service delivery plan development and approval process to the new page.
  • · Clarified that the lead agency cannot approve a temporary service delivery plan for a person who does not have a previously approved CFSS Individual Service Delivery Plan, DHS-6893P.
  • We removed extended PCA from CFSS Manual – Extended CFSS. For more information, refer to the Aug. 18, 2026, eList announcement.

We updated CFSS Manual – PCA, CFSS and CSG enhanced rate/budget to clarify how people using the budget model can use the enhancement.

We removed CFSS Manual – PCA communication to person’s physician. As of Oct. 1, 2024, lead agencies are no longer required to send communication to the person’s physician.

We added information about using the optional form CFSS Individual Service Delivery Plan (short version), DHS-6893Q (PDF) to CFSS Manual – PCA/CFSS service delivery plan (care plan).

    Forms and documents

For a list of all forms and documents currently available, refer to CFSS Manual – Forms and documents.

    MMIS system updates

DHS continues to test and fix bugs in MMIS. Counties/Tribal Nations can use Service Agreement and Screening Document (SASD) Support Team Portal, DHS 3754 to report issues to DHS.

    Resources

DHS continues to add new consultation services providers to DHS – CFSS consultation services providers as they complete their enrollment requirements.

    Reminders for consultation services providers

    DHS reminds consultation services providers:

  • · They can help the person draft their CFSS service delivery plan for the next year before the person’s reassessment.
  • · They must ensure the person understands they may need to adjust the CFSS service delivery plan based on the reassessment results.
  • · They must review the reassessment results before they submit the CFSS service delivery plan to the lead agency for approval.
  • DHS also reminds consultation services providers they must:

  • · Complete all fields in the provider agency and/or financial management services (FMS) section of the CFSS Individual Service Delivery Plan, DHS-6893P.
  • · Confirm all entries match the provider’s information.
  • The lead agency uses this information to identify the correct provider when they authorize services.

    Reminder for lead agencies

DHS reminds lead agencies to complete the following steps in MnCHOICES to assign a person’s consultation services provider to their record:

  • · Select “assignments” in the person’s record.
  • · Select the location tab.
  • · Use the new location assignment to assign the consultation service provider as the assignment type.
  • · Select the provider from the drop-down menu.
  • The consultation services provider then assigns specific staff.
