    Documentation and Verification – Holding & Local Flag

    Who collects the documentation?

    The employment counselor.

    Who provides daily supervision?

    The employment counselor.

    What information needs to be included in the verification?

    Information about the situation that has created the status.

    Which documents meet the requirements?

    Document in case notes why the holding or local flag activity is being used.

    For Holding – Sanction:

  • · Copies of notices of intent to sanction.
  • · Case notes documenting the reason for the sanction and attempts to contact the participant to cure the sanction.
  • · For FSS participants: Family Stabilization Services Pre-Sanction Checklist (DHS-6075) (PDF).
  • · A copy of the status update form sent to the eligibility worker requesting sanction status to be coded on MAXIS.

  • For holding – Child Under 12 Months:

  • · If the employment counselor processed the request for the exemption: Copy of status update sent to eligibility worker requesting coding for exemption or case note that exemption form was sent to eligibility worker.
  • · If the eligibility worker processed the request for the exemption: Case note that the child under 12-months exemption status is coded on MAXIS.
